Have you ever wondered what would happen if you had a serious accident or illness and were unable to speak for yourself? Would your family know what medical interventions you wanted? Would they be able to ensure that your wishes are met?

Arwyn Elden, from Empath Choices For Care, will offer a class on advance care planning for FCM members and their families and friends. Participants will be given the opportunity to explore personal values and preferences about how you would like to be cared for if you were unable to care for yourself. This valuable tool can give you and your family the confidence and peace of mind to carry out your wishes.   

You can register to attend this class in-person at the Tampa Practice Center or if you are an FCM Memer, you can register to attend via Livestream.  Please include the names of guests that will participate with you.

The objectives for this class are for participants to:

• Assess values to determine needs at the end of life
• Learn how to initiate essential conversations about quality of life and goals of care in regard to medical decision-making
• Understand how and why we need to choose a health-care surrogate
• Review how to complete and utilize a living will
